The Case Manager will support consumers in achieving greater independence and improved quality of life by coordinating care, advocating for services, and ensuring ongoing access to necessary resources. **Key Responsibilities:** - Provide direct case management services to individuals diagnosed with SMI, including assessment, service planning, referral, linkage, monitoring, and advocacy. - Develop and implement individualized service plans in collaboration with consumers, families, and interdisciplinary teams. - Assist clients in accessing mental health, medical, housing, employment, educational, and social support services. - Conduct home visits, community outreach, and transport clients to appointments and community resources as needed. - Monitor consumer progress and update service plans accordingly. - Maintain accurate and timely documentation in compliance with agency and regulatory standards. - Participate in multidisciplinary team meetings, supervision, and case reviews. - Ensure coordination of care across service systems to support holistic client wellness.
