We are seeking a compassionate and skilled **RN Admission Care Liaison** to join our South Region team. This role is ideal for nurses who are passionate about hospice care and thrive in a patient-centered environment. ### What You Need to Know About This Role - This is a **field-based, driving position** supporting Ohio’s Hospice South Region, serving the greater Miami Valley and surrounding communities. - Schedule: **10:00 a.m. – 10:30 p.m.** Rotating 3-week schedule - Holiday rotation required - Candidates must be comfortable traveling throughout the service area and providing care in a variety of settings. ### Position Summary The RN Admission Care Liaison plays a vital role in ensuring patients receive timely access to hospice services. In this position, you will conduct hospice admissions, coordinate care, educate patients and families, and connect them with the resources needed to navigate end-of-life care with dignity and support. Success in this role requires strong clinical judgment, excellent communication skills, compassionate patient advocacy, and a genuine commitment to hospice and palliative care. ### Key Responsibilities - Complete hospice admissions efficiently and accurately. - Assess patient needs and coordinate appropriate hospice services. - Educate patients and families about hospice care and available resources. - Develop and maintain positive relationships with patients, families, referral sources, and healthcare partners. - Accurately document all patient interactions and services within the electronic medical record at the point of care. - Meet organizational performance expectations, including maintaining a 75% admission conversion rate. - Ensure compliance with all hospice regulations, policies, and standards of care. ### Qualifications #### Required - Graduate of an accredited nursing program. - Active, unrestricted **Registered Nurse (RN) license in the State of Ohio**. - Current **CPR certification**. - Strong computer skills and proficiency with electronic medical records. - Valid driver’s license, reliable transportation, and a safe driving record. - State minimum automobile insurance coverage. - Ability to travel and drive during daytime, nighttime, and adverse weather conditions. - Ability to successfully pass a background check and 10-panel drug screening. #### Preferred - Associate Degree in Nursing (ASN) or Bachelor of Science in Nursing (BSN). - Two years of acute care nursing experience. - Certification in Hospice and Palliative Nursing (CHPN). **CHPN certification is preferred and required within two years of employment.** Eligibility begins after completing 500 hours of hospice or palliative care experience within the previous 12 months. ### Benefits & Perks We believe taking care of our team is just as important as taking care of our patients.
