This role is central to ResidentRx�s proactive, pharmacy-driven model for medication access. Responsibilities Monitor incoming medication orders throughout the day to identify acute and time-sensitive prescriptions, including new admits, antibiotics, pain medications, insulins, and steroids. Initiate the acute order process immediately upon identifying a qualifying order, rather than allowing it to wait in the general queue. Review acute orders for clinical completeness, flagging missing information, potential interactions, or prior authorization requirements close to the time the order is received. Coordinate with prescribers, community staff, and pharmacists to verify orders, resolve issues, and obtain any required authorizations. Arrange backup fill and delivery logistics for acute medications, targeting same-day delivery to the community when clinically appropriate. Communicate proactively with community teams regarding order status, expected delivery timing, and any delays or issues that arise. Maintain accurate documentation of all acute order activity, including timestamps, actions taken, and outcomes, to support operational reporting and quality improvement. Track and escalate orders that encounter barriers such as stock-outs, prior authorization holds, or prescriber response delays, working to resolve them before end of day. Collaborate with the pharmacy team to distinguish between acute orders requiring immediate intervention and routine orders that follow the standard nightly UPS delivery process. Support the onboarding of new communities by helping establish acute order expectations and communication workflows. Participate in the Quality Management Program by reporting quality-related concerns through appropriate channels. Reporting events related to patient safety is mandatory. Perform all other duties as assigned by the Pharmacy Manager or operations leadership. Core Competencies Proactive Mindset � Anticipates problems before they escalate.
