This role includes hospital rounding, dialysis management, and outpatient follow-up care. Responsibilities Conduct comprehensive patient assessments and develop treatment plans Manage patients with chronic kidney disease (CKD), end-stage renal disease (ESRD), hypertension, electrolyte disorders, and other nephrology-related conditions Perform hospital rounds and coordinate care with physicians and interdisciplinary teams Monitor dialysis patients and assist with treatment management Order, interpret, and communicate diagnostic tests and laboratory results Provide patient and family education regarding disease management and treatment plans Document patient encounters accurately and timely within the electronic medical record Participate in quality improvement and patient care initiatives Qualifications Required Master's or Doctorate degree from an accredited Nurse Practitioner program Current Texas Nurse Practitioner license or eligibility for licensure Acute Care Nurse Practitioner certification (AGACNP or ACNP) required Current DEA license or ability to obtain Current BLS certification Preferred Nephrology, hospital medicine, critical care, internal medicine, or dialysis experience New graduates with relevant clinical training and a strong interest in nephrology are encouraged to apply About Panoramic Health Panoramic Health is the nation's leading integrated kidney care organization, partnering with nephrologists across the country to improve outcomes and transform the delivery of kidney care.
