# Job Posting Title **ACT TEAM LEAD** ## Job Description The ACT Team Lead provides clinical and operational leadership for an Assertive Community Treatment (ACT) team. This position is responsible for overseeing service delivery, regulatory compliance, staff supervision, program performance, and fiscal management. The ACT Team Lead ensures high-quality, person-centered services are delivered to individuals with serious and persistent mental illness while promoting recovery, community integration, and overall well-being. The Team Lead serves as a mentor and resource to staff, coordinates treatment planning and service delivery, maintains compliance with applicable regulations, and supports the overall success and sustainability of the program. * * * # RESPONSIBILITIES OF THE JOB AND ESSENTIAL ELEMENTS ## Direct Duties - Provides clinical and operational leadership for the ACT team. - Conducts and oversees comprehensive assessments and treatment planning for individuals admitted to the program. - Evaluates individuals on an ongoing basis and coordinates treatment interventions to meet changing needs. - Assigns caseloads and service responsibilities to team members. - Maintains an active caseload and provides direct services as needed. - Supervises and supports multidisciplinary team members to ensure effective service delivery. - Assists in recruitment, interviewing, selection, onboarding, and retention of personnel. - Provides orientation, training, coaching, mentoring, and ongoing support to staff. - Conducts regular performance evaluations and provides corrective action when necessary. - Organizes, schedules, and distributes work assignments among team members. - Communicates agency policies, procedures, and program expectations to staff. - Ensures staff adherence to organizational policies, procedures, and regulatory requirements. - Ensures 24/7 crisis intervention services are available to program participants. - Guides staff in accessing and coordinating community resources and support services. - Assists staff with service coordination, problem resolution, rehabilitation planning, and support network development. - Supports implementation of recovery-oriented services, rehabilitation interventions, and peer support services. - Reviews and monitors caseload activity and service delivery outcomes. - Oversees maintenance of medical records and documentation to ensure regulatory compliance. - Ensures treatment plans, assessments, progress notes, and other required documentation are completed accurately and timely. - Develops and maintains staff schedules to ensure adequate program coverage. - Collaborates with financial and administrative leadership to support fiscal sustainability of the program. - Monitors program expenditures and promotes effective resource utilization. - Maintains census levels necessary to support ongoing program operations. - Participates in meetings, trainings, and quality improvement initiatives. - Documents time worked accurately. - Other duties as assigned by management. * * * ## Other Responsibilities - Establishes and maintains effective working relationships with program participants, families, staff, community agencies, and referral sources. - Promotes a recovery-oriented, trauma-informed approach to service delivery. - Demonstrates leadership, professionalism, and sound clinical judgment. - Participates in quality assurance and quality improvement activities. - Assists with program audits, licensing reviews, and accreditation activities. - Supports staff development and professional growth. - Communicates effectively both verbally and in writing. - Carries out responsibilities in accordance with organizational policies, procedures, and applicable laws. - Maintains confidentiality and protects all protected health information. - Observes work hours and is punctual for meetings, appointments, and assigned responsibilities. - Other duties as needed. * * * # ADHERE TO AND FOLLOW - The By-Laws of…
